End of life became a strategic priority for NHS Worcestershire in 2008/9, at which point, of the estimated 60% of patients who expressed a wish to die at home, only 20.6% actually realised this preference7.

The QIPP agenda, Department of Health National End of Life Care Strategy (2008)1, Kings Fund top 10 priorities for Commissioners 20112, Palliative Care Funding Review 20113 and the more recently published Recommendations for end of life care to new commissioners4 reinforce the need to ensure that end of life is a top priority, not only to improve the quality of care for patients at the end of life but also to ensure better value for the NHS.
